TS In-Person Class

To request a Transplantation Science class, please complete & submit the form below .

A Public Education Coordinator will contact you within approximately five business days to confirm details. Please make sure you have read the program FAQs before submitting your request.

Please keep these things in mind:

  • Please submit three possible dates you would like to participate in the program. We will make every effort to accommodate the first choice, however, our schedule fills up fast. Educators will not be available Dec. 11-31, 2023.
  • Requests for classes should be submitted 3-4 weeks in advance of the date(s) requested. We schedule classes in the order in which requests are received.
  • Please submit one form per teacher with each teacher’s email address. We prefer to email the confirmation to each individual teacher to keep communication clear.
  • If you teach less than 50 students and are in a small school more than 100 miles from Casper, Colorado Springs or Denver, please consider requesting the Transplantation Science Traveling Trunk for your students.

  • Class Information

  • If multiple teachers, please choose one classroom. It is time-consuming to move materials from room to room, especially with a short passing period.
  • (e.g. visitor parking; north side)
  • (e.g. main entrance on north side of the school)
  • (Click "+" to add additional periods. e.g. Period 2: 9:06-9:56; Period 3: 10:00-10:50, etc; )

  • If you have more than 35 students or a special schedule, please include it in the questions/comments field.
  • Please check the box if you would like to use any of the following programs for your students. Please add more information in the comments (e.g. how many ELLs you have, etc.)

  • Contact Information

  • We need an after hours phone number in case we need to cancel. This will only be used if Educator is unable to make it or we need to adjust for weather cancellations
  • Sometimes our emails don't get through district firewalls. Please check SPAM folder or add transplantationscience@donoralliance.org to your contacts.
