Research Application Form

All requests for tissues, organs, human cells, human biological material (HBM), and data are reviewed for research validity and procurement/preservation feasibility. Upon approval, Donor Alliance will develop an individualized protocol for the procurement, preservation and distribution of the agreed upon organ/tissue/specimen or data transfer based on the information provided in this application. For approved applications, a material transfer agreement or data use agreement will be required prior to implementing the research project. Please answer all questions as completely as possible.
