Transplantation Science In The Classroom

Request an In Person Class

To request an in-person Transplantation Science class, please complete & submit the form below.

A Public Education Coordinator will contact you within approximately five business days to confirm details.

Please keep these things in mind:
  • Please submit three possible dates. We will make every effort to accommodate the first choice. Educators will not be available the last two weeks of each semester.
  • Submit requests 3-4 weeks in advance of the date(s) requested.
  • Please submit one form per teacher with each teacher’s email address. We will email the confirmation to each individual teacher.
  • In-person classes are available to schools located in Northern Colorado, Southern Colorado, the Colorado Western Slope, and the state of Wyoming. Schools outside these service areas are encouraged to request a Traveling Trunk instead.

  • Class Information

    Please submit 3 possible dates 3-4 weeks in advance.
  • 3-4 weeks in advance, please (Click "+" to add additional dates.)

  • If multiple teachers, please choose one classroom. It is time-consuming to move materials from room to room, especially with a short passing period.
  • (e.g. visitor parking; north side)
  • (e.g. main entrance on north side of the school)
  • (Click "+" to add additional periods. e.g. Period 2: 9:06-9:56; Period 3: 10:00-10:50, etc; )

  • Contact Information

  • We need an after hours phone number in case we need to cancel. This will only be used if Educator is unable to make it or we need to adjust for weather cancellations
