To request a Traveling Trunk Transplantation Science class, please complete & submit the form below.

A Public Education Coordinator will contact you within approximately five business days to confirm details.

Please keep these things in mind:
  • Please submit three possible dates. We will make every effort to accommodate the first choice. Educators will not be available the last two weeks of each semester.
  • Submit requests 3-4 weeks in advance of the date(s) requested.
  • Please submit one form per teacher with each teacher’s email address. We will email the confirmation to each individual teacher.
  • The Traveling Trunk will be shipped to your school the week of the scheduled class and the teacher is responsible for shipping the trunk back. Postage is paid by Donor Alliance.
  • If you are within 100 miles of Denver, Colorado Springs, or Casper Wyoming, and you would like an in-person class, please submit a request for an In-Person Transplantation Science class instead.
