Donor Memory Cards

In gratitude of your loved one’s gift of donation, Donor Alliance would like to offer your family complimentary Donor Memory Cards. The cards measure 2.5 inches wide by 4.25 inches in height, they are printed on card stock and then laminated. If you would like to order these cards in honor of your loved one, please fill out the form below. The photo you select may be cropped or have damages to the photo itself (scratches, dust, etc.) digitally removed to be better viewed on the cards. The quote, prayer, poem or eulogy you provide for the back of the cards will be individually typeset to make these cards a personalized and lasting tribute to your loved one. You will receive a confirmation email with your order details, and the cards will be printed and shipped within one business day. There is no charge for ordering Donor Memory Cards.

Please be aware that this form will not generate a preview of the card design.

It will take 2-4 business days for the cards to arrive. If you need the cards by a certain date, please indicate this on the form below and we will make every effort to have them arrive on or before the requested date.
